K registration

I have a question. My child's annual check up is in mid-April, & k registration opens at March 18. Can I request SR-6 form filled out by pediatrician now through online request or should I have to wait till after the annual checkup (i.e. after mid-April) to request SR-6 form filled out? I need all completed forms before I can register K.

A couple years ago, when we registered, the secretary told us just to leave it off and email it to her after the appointment (over the summer for us). I'd call the school and check though.

You can turn it in once you get it.

Sometimes it only needs to be within a year so you could use last years physical and just have them fill out the form.

The doctor will probably fill it out now as long as you have had an appt in the last year.

Or have them fill it out after the appt. Either way is fine.

You do not need the health forms until school starts. As long as you submit the basic required info, like photos of parents IDs, kid’s birth cert etc, your registration will go through.

You could just turn in the checkup form later, or you could wait to submit all the paperwork after you have the annual exam. Just call the school now to let them know you'll be enrolling your kid.
