Transcripts for college - do the work habits grades show up?

No. But if you apply ED or sometimes EA, if college requests first quarter grades, you submit that first quarter report card, which does have those codes.

The transcript only includes the semester course grades.

Most of our teachers leave those blank

No. Report cards only have stuff from that term. Transcripts only have grades.
