Do they include the report cards when sending the file to the central committee? Are they expecting all 4s in the report card for consideration for LIV?

Yes, 1st grade and first quarter 2nd grade report card is included. I don’t know that they’re looking for all 4s per se, I think they’re just looking for consistency in the story the packet is telling.
