Adhd impulsivity and immaturity

My rising 6th grader has adhd and he is incredibly immature and impulsive. He also has been diagnosed with anxiety and ocd. I suspect mild autism, but he has been evaluated by a neuropsych and a developmental pediatrician and that was never diagnosed. He is obsessed with anything bad or inappropriate. He constantly talks about drugs, guns and sexually inappropriate topics. I think he is introduced to these topics by middle school peers but he is so immature that he can’t handle it. He pretends he is drinking and doing drugs with non alcoholic beverage and sugar. It is bizarre

His therapist says this is attention seeking and that is part of it, I agree. I also think impulsivity, immaturity and social deficits are a huge part of it. He is medicated, but I don’t think it’s doing enough. However, we’ve tried several meds and he can’t tolerate much. His behavior alarms me. I’m at a loss for how to handle him

I can totally empathize. DS has anxiety and ADHD, and I get the sense that zeroes in on language that gets a reaction from people and uses inappropriate words to seem less like a "little kid." I worry because he will be going to middle school next year and I know he is the kind of kid that could be easily manipulated by others to do stupid things and get in trouble for them. Any suggestions on how to manage this? Just talking to him about these things doesn't seem to get through to him.
