Infertility Support and Discussion

I have posted before, and I just want some ideas as to what else I can do, what other tests to ask for.
I have a 2yo. I had a missed miscarriage last October, emryo measured 6 weeks. I got pg again after 4 months TTC. I miscarried again, missed mc, embryo stopped growing at 6 weeks again, 2 days after we saw a heartbeat. Had a D&C this time and they tested the tissue, the embryo was genetically normal, and my testing came back normal too. They did a clotting panel and turns out the only thing is compound heterozygous MTHFR for the A and C genes. Was told to take a lot of folic acid and baby aspirin. Was told Lovenox was not necessary. I tested once for very high FSH, but I don't think this is a problem as I ovulate every month, and if the embryo was normal the egg was probably ok too right? Any ideas? Not having an answer to the two losses makes it so much harder, I wish there was an explanation.... Thanks. |
| How's your husband's sperm? Sometimes a fetus stops developing if it lacks genetic material provided by the sperm. So even the egg was fine, the sperm might no have been. |
| We haven't tested it, but if the embryo is normal then sperm is probably ok right? |
| Not necessarily. If there were no chromosomal abnormalities, then the egg was OK, but it doesn't mean that the sperm was. I would send your husband for a sperm test. |
| Check earlier thread about sperm fragmentation and info on testing on this listserve. Found it enlightening. |
I this is not right. When testing for chromosomal abnormalities they test the embryo which results from the fusion of the egg and sperm - they test to see if the embryo is diploid for all of the chromosomes so both the sperm and egg were fine if they didn't find anything. I would be relieved that they found that you are compound heterozygous for the MTHFR mutation - this would explain the miscarriages and give you a treatment plan to move forward. |
| What sex was the chromosomally normal embryo? If it was a girl, then you can't rely on it having been a chromosomally normal embryo since they could have been testing your tissue and not the embryo's. If it was a boy, then you can rely on the chromosomally normal result. |

It was a female. Ob said they also did genetic testing on me and that it was normal, wouldn't they be able to tell if the tissue was mine or the embryo's?
15:05 I am kind of relieved but feel baby aspirin and folgard can't do that much, in my mind lovenox will work,,, The embryo was chromosomically normal so that would mean both sperm and egg were ok. |
| Unfortunately, at such an early stage they cannot distinguish the embryo tissue from your own tissue. I had a m/c at 8 weeks and was told that the normal female was inconclusive for that very reason. Unless they did specific karyotyping of you and the embryo, which is not usually done, there is no way to tell whose tissue it was. |
They do different things. The folgard will compensate for your low folic acid status because of your MTHFR genotype. The lovenox is something that is tried for women with multiple miscarriages - its hard to know if that will really work for you since that was not the finding from your tests. You can push your doctor to prescribe that rather than taking the baby aspirin. Your doctor probably only mentioned taking the asprin since it can't hurt and may even help - not sure what the side effects of lovenox are. |

Hmm ob was pretty convincing when she said the embryo was normal. I guess it does make sense.
I was told by a hematologist that lovenox is as safe or safer than baby aspirin so I might push for that, for psychological reasons if anything... |
| Have you and your husband also gone through karyotyping? When I miscarried, the fetal tissue test came back inconclusive so they couldn't say one way or the other if the embryo was chromosomally abnormal or not. They thought the karyotype might reveal whether or not we were passing on a chromosomal abnormality but we both cleared and continue to try. So sorry for your losses - I know how hard that is. Best of luck to you! |
| I don't have any insight. But I am in the same boat as you. Have a two year old and two miscarriages. All tests came back normal. |
| Same thing happen to me I have 3 kids I am 34 years old just had a lost in feb and last week had d&c doctor said just bad luck but will do test after 6 weeks don't understand why I have to wait. Would like to try again but want to see what is wrong frist. |

Absolutely INSIST that they take a look inside your uterus to make sure the D&C didn't leave any scars or that you don't have fibroids or whatever. I'm 4 miscarriages in and my new RE just discovered some scarring. We'll never know how long it was there, but it can cause miscarriage and I regret so much that I didn't demand the testing earlier. I think they usually start with a Hysterogram.
Sorry for your losses. |
