How often is head to head record used as a primary tiebreaker over goal differential?

Also, NFL has a multi-team tiebreaker, where once the first team is picked, they revert to the first tiebreaker to break further ties, so a head to head win will advance a team even though it lost on a tiebreaker like conference record. This would be more of an issue in soccer, where a team can be 3rd in goal differential but advance over the 2nd place team. I saw a team that was in a tournament with this revert rule, and to advance to playoffs, the team deliberately gave up a goal.
