… or can the doctor fill it out based one the most recent physical, which was done within the 1-year window?
I called the doctor’s office, but they didn’t know, and I found the form unclear. |

| Yes from the most recent physical so long as it’s more than a year old. Also the physical will expire at the one year post physical point and you will have to update it if your child is still playing sports. Otherwise your kid gets benched. |

It’s not a big deal. This year I had my daughter’s done at Fast Track. The doctor did the whole form, even the parts that the kid has to answer. It took like 15 minutes including physical and left with form in hand. |
