Are they actually refundable on demand? No reason needed? Or is it “travel insurance” that’s written in a way that’s impossible to actually use?

No. Worst case, you can either get a refund as airline credit; or you can change to a future flight (if you haven't departed yet) and just pay the difference.

It depends on the destination and other terms (e.g. is change allowed with penalty? Is cancellation allowed with penalty?). If the tickets are non-changeable non-cancel label even with penalty, then yes, I’d pay more. If cancellation or change is allowed with penalty, then I wouldn’t pay extra at this stage.
