4yo and night vomiting

My 4yo vomits once a month or so at night while he is sleeping. It’s usually around 2-3 hours after going to bed and is preceded by coughing. I can’t tell if it’s the coughing that causes the vomiting, or it’s coughing because his body needs to vomit. It always just happens once and then he is fine and goes back to sleep after being cleaned up.

I have tried to figure out if there is a pattern with what he eats, but I don’t think so since he is so picky and eats the same things a lot, so don’t think it could be an allergy. I do notice that his food is not digested yet when it happens, and honestly it looks like he doesn’t chew well when it happens (sorry for tMI but he had max and cheese for dinner and threw up whole pasta shells).

Has anyone experienced this? It has probably been happening for a year and a half. I mentioned it to the doctor once but she wasn’t concerned.

Acid reflux by any chance? Lactose intolerance?

I would get a second opinion
