Here's one example. Sorry for the format is jacked up.
1. What do you consider the most important achievement(s) of your child during the high school years? 2. Describe your child using 4-5 adjectives. 3. How has your child overcome adversity, shown strength, courage, or resiliency? 4. What do you consider to be the personality and character traits that make your child special (communication skills, willingness to take risks, social skills, leadership style, etc)? Whenever possible, please add anecdotes (specific situations, stories). 5. Are there any other personal or unusual circumstances which have affected your child’s educational or personal experiences |
