Questionnaire for HS counselor to write letter of rec

Would you mind copying and pasting the parent and student questionnaires your child filled out for the HS counselor?

Why? What is your purpose with this? Each high school has different versions. How are random sets of questions useful here?

Can you think of no usefulness in this? Let’s see:

Use it to prepare a sheet of info for letters of rec from other sources
Use it to give to a kid who has asked for a letter of rec from you
Use it for a counselor, esp at a school with a weaker admin
Use it as part of a presentation to HS students

That doesn't involve the knee-jerk criticism of strangers.

OP- you can Google high school names and often on their Guidance Counseling department page or College prep page, they will have this form available. Good luck!

I no longer have access to the parent google doc that askes questions. And, we never saw the student questionnaire...

Here's one example. Sorry for the format is jacked up.

1. What do you consider the most important achievement(s) of your child during the high school
years?

2. Describe your child using 4-5 adjectives.

3. How has your child overcome adversity, shown strength, courage, or resiliency?

4. What do you consider to be the personality and character traits that make your child special
(communication skills, willingness to take risks, social skills, leadership style, etc)? Whenever
possible, please add anecdotes (specific situations, stories).

5. Are there any other personal or unusual circumstances which have affected your child’s
educational or personal experiences
