US Passport - City and State Issued?

How do I find out "where" my US passport was issued? Mine only says UNITED STATES/USA

I'm trying to renew my Global Entry - the application is asking for "city and state".

I thought the city and state question was about where you were born? I renewed my GE a few months ago and that's the only question I recall that required a city/state answer.

Doesn’t it say issuing agency or something on your passport?

Yes, it says Dept of State. I'm just going to put that down, see what happens. Thanks!
