Mother-Daughter Survey

Hi! I am currently a college student performing research on the health of mothers (ages 30-45) and their daughters (ages 8-12). This is a great opportunity to fill-out this survey with your daughter and explain to her the benefits of good nutrition and health. Please email me at [e-mail] to obtain a copy of this survey.
