what are the out-of-school-time day camps that require covid vaccination for those who are 5 and up?

these are the ones i've found so far:

audubon naturalist society and imagination stage require vaccination of campers.

nature lab is undecided and said check back in late january for a decision.

casa lala does _not_ require vaccination of campers, but reports that all their staff is voluntarily vaccinated.
