Question on completing a w2

I have someone who comes and cleans my house each week. I live in DC and she lives in MD. I am attempting to complete her W-2 for taxes for 2012 and I am stuck.
I have entered all of the simple items - her wages, Social Security Wages, SS tax withheld etc.

I am stuck on entering the state and local wages section.

Do I enter state wages as DC -since this is where she worked? or MD since this is where she lives?
If the answer is MD, the next box I need to fill in is my EIN. I have an EIN for DC - not MD. Do I need to get a MD EIN?

Hopefully someone has been in this situation and provide guidance.

no - we only withheld SS and FICA.

Box 15 is the State and State EIN for the employer. In your situation, this would be DC. You do not need a MD EIN.

Are you actually withholding local taxes for her? Employers of household employees normally only withhold FICA and/or Federal taxes. If however you have generously opted to withhold state and local taxes, those need to be calculated based on the employee's state/locality of residence. So you would list DC and your DC EIN in Box 15, but boxes 16-20 would all be based on MD and the jurisdiction of residence of your employee. Sorry, I see now that you did state that you only withhold FICA. Because of that, boxes 16-20 would be left blank.

W-2 is to report what her income was from you and what you withheld and remitting for taxes - Fed, FICA, and state. If you didn't withhold state taxes, you would put zero withholdings for the MD state taxes because you didn't withhold and remit anything to MD.

I still have this same question, I employ a MD resident in DC, am not withholding any state tax. I have a federal EIN, I called DC tax and they said there is no separate DC EIN, it's the same as the federal (not sure this guy was the brightest bulb so please correct if wrong)...

but question remains- DO I LEAVE BOXES 15-20 blank or do I put in wages in box 16 (state) and what about box 18 (local)???

1. EIN also known as FEIN is the federal id. I know MD issues a separate id number for state taxes and unemployment.

2. All the info on the W-2 pertains to the place of employment.

3. Even if you don't withhold taxes, there is a form that you fill out , In MD it is called the reconciliation form (don't remember the number).
