But nobody can demand to keep their job despite not getting it, either. At will means, well, "at will."

I would talk to the nanny just like the OP did. Some nannies are scared of new vaccine, mainly due to false information being spread on spanish-speaking social media. Even if parents are vaccinated, their kids are not, so the kids are at risk, or maybe having unvaccinated nanny is preventing family from visiting grandparents. Lots of reasons to want nanny vaccinated. I agree that no vaccine would mean no hire. I myself is a nanny who spent last year at home in fear of covid, thanks to my official employment I was getting unemployment benefits, and I would only start working for fully vaccinated family who still wear masks in public.

Good job. While people can choose to not get vaccinated you can choose who you employ!

We just went through this. Our nanny was hesitant about getting the vaccine and kept putting me off in regard to when she was going to sign up. Finally, on Friday, we sat her down and told her how much we liked her and how our DD loved her but we couldn’t keep her in our employ without being vaccinated. She took it well and signed up for an appointment.

I had to quarantine a few times over the last year. It was a huge inconvenience.

OP, I would not keep a nanny who refused to get vaccinated.

And what about her husband? She may not get sick, but she could still be a carrier and infect the kids.
