I didn't answer because I really don't get the point of your question. The top tier of luxury care is a nanny that gets paid six figures and meets the greater demands of the family. Of course most families can't afford that and that's not the standard nanny. There are also concierge service doctors, and yet most people manage to get by with a typical physician. Why are you so obsessed with celebrities and the uber-wealthy?

Hiring a nanny period is the top tier. Coming to your home is meeting the greater demands you fool. It’s private, in home, one on one care for your child. Why would that even be affordable? Someone is coming to your house to raise your kids. Does that sound like a service that would be cheap? You are absolutely right that not everyone can afford a nanny or in home physician (again, who are these services aimed toward). That’s why there is daycare and places like Kaiser. You seem to think you deserve a nanny even if it means paying them low. I don’t understand why you are, oh so offended, when someone says you cannot afford a nanny. You can’t. Go to daycare. You are quoting care.com! Now that is funny. Care.com is the absolute worse resource. Not to mention the families on there are cheap with high expectations (which thank you for proving). Also, care.com doesn’t exactly distinguish between babysitter and nanny or temp care or long term care. You don’t need any credentials to post on there. You don’t even need a background check. Anyone who believes $15/hr is a good wage is cheap and should be looking into a home daycare. Even an excellent daycare center would be too much. The middle class wants want the upper class has, this is not a secret. Nannies were never meant for the middle class and anyone with basic knowledge knows that, hence nanny shares where you can pay $12/hr.

They hired a nice lady to take care of me and my siblings. No way they could afford to pay her a month's salary as a bonus. The idea of them buying a luxury car instead of giving her a bonus makes me laugh. They drove 15 year old used cars and we wore thrift store clothes. The nanny was well-loved and respected and considered a part of our family and came to all our family functions even after we grew up. I can guarantee you she never felt used or disrespected when we couldn't pay her a huge bonus at the end of her years with us. Maybe that's because we didn't treat her as a member of some lower class. And I'll bet you anything my parents paid our nanny a higher percentage of their own salaries than many of the posters here who are suggesting that if you can't give your nanny a five figure bonus then you can't afford a nanny.

I don't even have a nanny and am only in this forum because I am searching for one. I can afford to pay a potential nanny more than most and I can afford generous bonuses, but that isn't at all the point.
