Because she referred to you as "the help"? Sorry, but you are a nanny. You *are* the help. You seem to have some issues with your title. That's fine. Don't like it? Go get an education.

My lawyer, my plumber and my doctor are all my *help*, as is my nanny. They all work for me, as long as they keep doing a good job. If they don't, I fire them, and get a better replacement.

Sorry, your doctor does not work for you! Neither does your lawyer but as you hire them and maybe put them on retainer, that is a little bit closer than with a doctor. You don't pay a doctor an hourly rate to do something for you. You pay the few they charge and work around their schedule of when they are free. You have to be an employer of the person to technically call them "your help".

This is absurd. If my doctor isn't working for me, while she's tending to my needs, who do you think she's working for?
Her pussy cat?

If I fire my nanny, or she quits, she has her other PT clients to maintain, just like my lawyer, plumber and doctor, do.

Your doc isnt working for you. If you fire your doctor, they will be alright. If you fire your housekeeper, she/he will have to look for a new job. I have quit a doctor before because he had shitty bedside manner, but he is still employed by kaiser.
