Part of the issue is that she insists on taking unpaid time! It's created a situation where she thinks she has unlimited time off as long as it isn't paid. It's really the burnen of finding backup care so often and that when she wants to leave just an hour or two early, It's extremely hard to find someone to come for that short a period.

Buckle down, OP.
No more unpaid time.
Only approve leaving early if A) she gives you a certain amount of notice and B) you can find coverage. Even then it should only be under certain circumstances, like a medical appointment.
Have a discussion, and update the contract to include clauses for these things. If she disagrees, she will have to find somewhere else to play. She isn't treating this like a job.
