I’m all about giving someone the benefit of the doubt and the first time maybe she didn’t know how to buckle correctly but clearly she just doesn’t care. I would fire her because you’ll always be wondering what other safety measures she’s just ignoring.

Frankly, I would have let her go that day. Your child has been unsafely secured in his carseat the whole time she has driven him.

This is a life or death situation. I would find another nanny, not acceptable at all.

Unless the child is a houdini who can unbuckle and get the upper arms off, there's no excuse. We typically are better about knowing whether a car seat is installed correctly and whether a child is fastened in appropriately (no puffy coats, mom), because this is literally our job, our reputation.
