The shot doesn't work perfectly. It just decreases your chances of getting the flu if you're around someone with the flu. If you get the flu you'll have to miss work, which would be an inconvenience to the family. It's convenient for you, given your fear of shots, if your family doesn't require it, but I think it makes a lot more sense for parents to request that their nannies get the flu shot.
I don't have a fear of shots, I just don't agree with injecting things into my body (aside from childhood vaccines) or taking medication unless it's absolutely necessary. This is not necessary. I do not get sick. I have a healthy immune system because I don't eat garbage or inject garbage into my system. I have taken one sick day in 15+ years of nannying. If families don't like that, they are more than welcome to pass me by.
And I am sure many employers will pass you by.
