I would not give her paid vacation or sick time if she is being paid $20 an hour. Her compensation is already on the high end and she should be able to plan accordingly for unpaid time off.

It's generally a good idea to offer PTO time in terms of hours rather than days. It avoids ambiguity in cases just like this. You can still specifically that PTO can only be taken in minimum increments of 11 or 5.5 hours if you want to avoid an hour of here and there.

Typical is 3 - 5 sick days; 1 week (5 days normally) of vacation that you pick and 1 week of vacation (again, normally 5 days) that they can pick.

Since you are paying very high for 1 kid you might be fine having the vacation be 1 week = 4 days instead.

I wouldn't do that, OP. You might want to take a day off to take care of something but still need childcare. I would stick with the two week standard. Holidays are different. Be careful about that and make sure you are specific in the contract. Nannies here in DC seem to automatically expect all federal holidays off, which can be hard if the employer doesn't get all those days off.

We offer two weeks vacation, one our choice and one hers. It accrues, but does not roll over into the next year.

We also offer five sick days.

Don't listen to the cheapest one! You are not asking their opinions about the rate! You are paying around the market! About sicks days, I got 10 and all the federales days!

We paid our nanny for 52 weeks a year. She took a week of her choosing off and then probably used about another week of sick.

However, our family travels a good amount. We personally took about 3 weeks off during the year, so she got about 5 weeks off a year.

We did have block out dates. Absolutely no time off during August/September (my busy season at work).

We were pretty loose with the vacation time, mainly because my job is very flexible and generally not so busy.
