I would leave the item laying near or on the washing machine. As you are leaving, mention to MB that "All of the laundry is washed, folded and put away. There is one item that I was unable to get cleaned. It is sitting on top of the washing machine. Have a great evening!"
She should look at it and be beyond mortified.
If she is upset that you didn't clean it, you are not working for good people.
Adult fecal matter is NOT part of any nanny's job description.

Put it in a plastic bag, tie up the bag, and mention it to the MB when she gets home. Be sure not to act indignant when you speak with her.

Men can be remarkably clueless and shameless about this stuff. The MB will probably be mortified and/or amused at his pig-like behavior but totally understanding of your desire not to deal with it. She can then choose to throw it away, clean it herself, or hand it to her husband, hopefully with a gentle reminder that he should become a little more careful about what he leaves in the hamper.

I would not touch anyone's poop drenched pants. Period. You are not his mother. I would say "Joe, I saw the soiled pants in the laundry room. I am sorry you were not feeling well. I do not handle adult feces but I can give you some tips for how to handle soiled laundry. In the future, I recommend a thorough rinse in the sink and to wash the item separately."
