I feel they do not respect me and the fact that I have other commitments besides them.
You're the nanny and they do not respect you. You have three choices: (1) take kid with you to appointment and charge them double time for the inconvenience. (2) Tell them if they are late again you have no choice but to leave child by himself. (3). Find a new job with people who do respect you.

So they’re not going to change.
Your options are to either leave and find another morning position OR don’t schedule things as close to your end time as you have been doing. You know they’re not reliable, maybe ask them if they want you to change your hours to 1pm? I know this can be hard with another job but like appts, schedule them on your day off or mid afternoon.

Maybe the parents are forgetful and need to be reminded again.
Not that this is okay but if they are otherwise excellent employers I might let this one slide one more time.

Even if this happens only a few times, keep in mind that like another poster stated, if the tables were turned you would likely be let go stat.
There should exist a mutual respect between you and your bosses.
If none exist, then it is much better for you if you leave.

For you, it IS a big deal because you are scheduling things after the time you're supposed to leave. If you were just going home to watch Netflix and eat chips, you wouldnt' care as much. So find a job with more punctual people.
