And sorry to tell you; but I have also never requiere to make up those hours. Her employer is just Cheap. It's not worth for her, keep working for them. To OP. It's better to start for a new position, if the things are going on that way.

It's not being cheap and as a "professional" you get leave and you take leave for doctor's appointments. They should pay her for days they don't need her but if she is scheduled to work and cannot that is what sick leave is for. It does matter if you are salary vs. hourly and you clearly don't get how professional vs. hourly jobs work.

I am a nanny and I am using my sick time to go to appointments. Once I have used it up, I do not expect being paid for missed days or hours. At my $32 ah rate, 2 hrs is just way too much to pay for nothinh. Again, I do have 7 sick days a year and I never been needing more to be honest. Just get your family agree on your paid sick time, op.

Do you have a contract? If you’ve Been a nanny for 11 years you should know to have a contact where these things are outlined

You should speak up. But I am wondering why you didn't talk about this before the hire you and you accepted the Job. Actually this is one of the most important things you should agreed All Paid guaranteed hours, your 52 weeks paid.

It's their business if they don't need your services sometimes. You will be there and you are there always ready to work. If they had family in town and give you those days off, of course they have to pay you. Without deduct anything from your pay check.

Regarding your Dr. Appointment. It's also another mistake you make. You should have stated what is the way you work and what arrangement were you were looking before signing a contract.

Unfortunately both of you never talked about this in your interview and your contract I'm guessing. It's better talk to them, and see if all this arrangement can be fixed; depending what they tell you; you decide if you would be willing to keep working like that.

I work Full-time for a family that has a 1 year old, started when DS was only 2 months. The two times i needed to go to doctor i took about 1-2 hours off and got back to work. The family deducted those hours from my paycheck. A few weeks ago, DM told me she had family in town and wont need me one day. I got those 8 hours deducted from my paycheck, also. In the 11 years working as a FT nanny none of my former employers never clipped me for medical nor days off given voluntarily by them. Have you dealt with this issue before? Would you email or speak directly to them that im not happy with that and want guaranteed hours. TIA for your advice!
