Mine is easy:
Unlimited sick leave. I haven’t taken a single day, but the contract is written such that I can take as much as I need. If I’m sick two days in a row, I need to present some form of note from a medical professional about the expected duration.
Pto: unlimited half days, no whole days unless absolutely necessary. In reality, I don’t take any, because I schedule things for outside my hours.
2 weeks vacation, one their choice and one my choice, and both are notified by email at least three months prior. Please see notify: I don’t have to ask, just notify with at least 3 months notice. The only proviso is that vacation weeks can’t be consecutive, so whoever notifies the other first doesn’t have to change. Vacation can be rolled over to the following year (no maximum number of weeks; however, weeks may not be taken consecutively) or paid out. Frankly, I usually get mine paid out, since I have guaranteed hours and families take at least two weeks per year; going to get groceries on their last day away is not a big deal at all.

I use a calendar and calculate PTO used and remaining on every paystub.
