I have looked at his anus, not super red, no obvious worms (with my eyes looking at it), and he has no diaper rash. He is potty trained, wears underwear all the time, except diaper at nighttime just in case. Is this something I should be concerned or take himnto his pediatrician? I am suspicious he may have a tear at anus tissuebecause he pops every 2 to 3 days.

What kind of Nanny are you?!

You are asking a Nanny forum MEDICAL questions. You are the exact reason we have a 'troll' on this forum complaining about how unexperiences and uncertified our career is!

Are you serious? News flash: nannies did not get training together or moms etc etc. she sounds concerned with her charge and you're bashing her. Someone sounds a little weird.

If you are the Nanny ask the Mom what she thinks. If she is normal she will say take him to the doctor.

My kid only poops every 2 or 3 days too, always has, and doenst have this issue.
