As the nanny, please do.
It is awful to feel invisible to frequently visiting relatives.
It's inappropriate. The grandparents are not your boss.

I am a career nanny who often interacts with grandparents. I am honestly curious as to why you feel this is inappropriate. I very often have received gifts, generally gift cards, and very much appreciate it! And usually I buy work clothes with the money.
To the OP, unless her boss opposes, I think it is a lovely idea.

Would it be inappropriate for me to give her a small gift of appreciation?

STOP. Give us a RAISE or a BONUS. We don't need small gifts, its actually a bit insulting, considering how much we have to put up with.
