Probably bad news or she didn't feel she could tell you as maybe she hasn't accepted it herself.

Or maybe she got terrible news and doesn't want to tell you. Or it turned out to be nothing and she decided not to go. Or she didn't have the right insurance/money and had to make another appointment. Or it was a lie from the get-go, and she decided to come to work after all.

It was fine to ask, but the fact that you're calling her non-response about her personal health needs "evasive" is weird. My response would have been, "Oh! Just let me know as soon as you know when you'll need the time off." And then I would have promptly forgotten about it.

She was probably constipated. She took a laxative and was able to poop so it is no longer an immediate concern but she is aware that she has ongoing issues with constipation and really ought to make a doctor's appointment one of these days. And she either didn't want to have a lengthy discussion with you about her bowel issues, or she didn't want to have a lengthy discussion with you about how she doesn't have health insurance and can't afford to go to the doctor. Or she didn't want to talk about it with you for both reasons.

I have had wonderful close relationships with most of the parents I worked for and I still can't imagine wanting to discuss my digestive issues with them.

You shouldn't have asked about it. When someone says they have a doctor's appointment your only response should be "I hope everything is okay; of course you can have the time off for a medical appointment; thank you for giving me advance notice."

I would avoid asking too many questions about medical appointments in the future. Some people may find that invasive.
