MB here. We do a cash bonus from the parents and a gift from the kids. We've only had one nanny, but she's been with us for several years. First year we did one week's pay, after that we did two weeks. We withhold taxes (but the bonus was just cash), so I think the amount we did was a round amount somewhere between net and gross.
For the gift from the kids, I usually try to spend around $50. I usually try to buy something that's more of a treat than strictly practical, but also from a store where she could return it and easily find something else (e.g., cute winter gloves from Bloomingdales, a gift set from Sephora, etc.).
We're not super wealthy (esp for the DC area), but we're very comfortable We also love our nanny and really appreciate her and want to show that.

Personally, I think a small amount of money and a gift related to her hobby is great. Including a picture of the kid(s) in the card is always nice too. I like getting those and putting them on my fridge or hanging them with my cards which I leave up all year until the next ones roll in. Last year I got one of the family with Santa which was fun.
You have never been given a week's pay as a Christmas/Year End bonus? I am honestly surprised. It is pretty standard. I had one very wealthy family that gave two weeks bonus.

Nanny here.
Cash is the best way to go + a little gift from your child.
Don't do a frame if you're unsure she will display it.
I've been a nanny to about 25 kids and I can't display everything I was given, at some point this is too much and it breaks my heart to keep these things in a drawer.

One week's pay for the first year is good - in gross/in cash is best. I love the times I received a framed photograph of my charge and me. Or something I mentioned in passing that my employer remembered.

Our nanny has only been with us a short time, but we love her so far. I'm already starting to think about Christmas because if I don't shop early, I won't get it done. I don't get the impression that my nanny is hard up for money, but a cash gift (like an Am Ex gift card) is still probably the best way to go, right? Is one week's pay standard? I would like to also do some more personal gifts. I know a few things that our nanny is interested in and could get her small gifts related to her interests. I could also get her a little something with my son's picture on it from Snapfish (any ideas what?). Is all this too much?
She (the nanny) knows that we (MB and DB) don't make a ton of money but have prioritized spending our money on a nanny because we think the job she's doing is super important. I want to treat her fairly and show her how appreciated she is, but I also don't want her to feel awkward about how much we've spent, so I don't want to go overboard.
