I am 58. I was embarrassed last year when my MB gave me a bonus check for my birthday, and relieved this year when she didn't bother. Two years from now when I hit 60, if my employers want to go all out for my birthday, that sounds great.
Maybe that is just me though.
It is nice to be thought of, just don't go bankrupt expressing the sentiment.
All birthdays are milestones. You made it through another year!

Have the flowers delivered to her house. Traveling with fresh flowers is always a mess and opening the door to a flower delivery is such a wonderful surprise.

My favorite gift was a $150 gift card to a spa I had mentioned liking. You can do a gift card or day off (I'm sure either would be appreciated) and the flowers. If you do that cake, have it at your house with the kids and send the leftovers home. I wouldn't want an entire cake hanging around my kitchen begging to be eaten!
