OP here. I just put "refer to resume and reference list" under employment history and left the rest blank. Then under pay requirements I put "pay negotiated based upon family's needs, schedule, location, etc.". We'll see if it is well received

Tell the agency it is none of their business what you were payed, OK that is snarky
The agency wants to see if you can follow directions, just put down that your pay was competitive, that is all they should or need to know

I'm surprised that they want my employment history on an application when my resume has the information. I'm not applying at McDonalds! They give this to potential families and it has my desired salary and all sorts of stuff that is family/position determinate.
