Just so that you are aware, page 4 of form 9141 is to be completed by the government, not the employer. You don't set the wage, you just fill out the other three pages, and you don't need a survey to do so.

And you can't find a nanny already in the US who speaks the language? Don't you have to prove that you tried to fill the position before you can bring someone else in?

Does anyone have experience completing a Form 9141 prevailing wage determination in order to hire a foreign nanny? I am hitting a road block finding information on the survey name and survey date of publication required to complete the form.
