Does she have any sick leave to use? Most people, in any job, will first use any banked sick or annual leave regardless of whether they are doing something elective or not. Treat it the same as you would if she was having surgery for any other reason.

So it isn't necessary for you, congrats. That doesn't mean that it isn't necessary for OP's nanny. You don't know her situation, you don't know what conversations she had with the doctor, and you don't know whether her insurance is covering it as medically necessary. Unless you have those pieces of information, how can you make that call?

Regardless of whether the surgery is elective or necessary, normal procedure is to use sick days, PTO and vacation. Whether you decide to offer paid or unpaid leave beyond that is up to you, OP. And if you think that she will be out for a while, hiring a new nanny is also valid. If the surgery is medically necessary and you terminate her, she does have a right to collect unemployment, and I don't think she would be required to look for a new position while she's recuperating. I would suggest a candid talk with your nanny, see what her thoughts are, and what you budget is, especially because you're probably going to want to hire a temp nanny.

Needed surgery is not elective. Generally breast reductions are done because they're needed.

You can easily look up how long recovery from a breast reduction takes online.

I would do what is done in companies. You have to use up all your vacation and sick days BEFORE you can go on short-term disability, which is probably what your nanny will need.
