I just did this with Maryland when we moved out of state. There is a form, but I've forgotten what it is. Call the business tax office.

We are sadly saying goodbye to our long time nanny as the school year starts. I am canceling my service with GTM, which I do highly recommend. They will charge me a one time fee to notify the state and the IRS to close our tax accounts. I'd rather just do it myself. Does anyone know how to do that? Is there an IRS form? MD form or form letter?
