I do not think it is necessary to gift both your nanny AND her new grandbaby.

I would simply tell your nanny "congratulations," but not give her anything as a gift.

When the baby arrives, you can always purchase something for him/her, then give it to your nanny to give to her daughter's child.

I don't think it's appropriate to blur the boundaries that much.

Even if getting that involved in your nanny's life was appropriate, it's not the first grandchild, nor the first child for that daughter. Unless the other grandkids are all one gender, and this is the first of the other gender, I wouldn't gift for anyone other than immediate family (ie. mother is immediate family, not grandmother).

One of my nanny's daughters is about to have her third child, which I think will be my nanny's eighth grandchild. Do people normally give a gift to their nanny when a grandbaby is born? If so, what? I do plan to get a gift for the baby.
