Plus, both last year and this year I worked for them on my actual birthday (and reminded them about it in advance). Is there anything I can do? It does feel like a slap in the face

It's not about what gift you get. Be grateful.

Nothing less than $50.

Remember what she does for you and your family is priceless.

I'm sure she would prefer cash and a sweet gesture from the kids (a card signed by them or drawing), at her age she must be thinking about retiring and some cash could help her I guess.

My previous nanny family usually gifted me with a Visa Giftcard, which can be used anywhere.

I was actually planning to get her a $100 gift card so that should be enough to buy something she'd like. I could do Target but I feel like she might like Nordstroms better, but I don't really know. I want to get her something she'd like since she really made an effort to get things my DCs like.

What about a cozy sweater or some fancy pjs? Cashmere scarf?

You sound very generous OP! When in doubt, cash is always awesome! Also, a card with a thoughtful note goes a long way too!

We gave our nanny $100 when she had been with us for 6 months. We'll give her the same this year though too.

I would do $50-75. In less you know she shops at Nordstrom, I would go somewhere more practical too. I prefer Target, Marshall's, TJ Max, etc. I got a gift card from Nordstrom and waited 9 months to use it. It wasn't enough to buy me much so I finally bought shoes for my child. Or, maybe $50 to Target, etc. and $25 to somewhere like Panera, Chippolte, or where she can use for lunches if she is out with the kids.

Is she over 21? Wine.
Does she drink coffee? Mug/travel mug and Starbucks card or fancy coffee
Does she have a kindle? Amazon gift card

My only (minor) issue with gift cards to stores is that sometimes I don't typically shop there, so I have to try and find something I like, and then I usually have to spend more than the gift card. So I'm spending money I wouldn't otherwise spend on a gift I wouldn't normally buy.
I'm more practical though. I'd rather target over Nordstrom

I give $25 if we've worked together for less than a year and $50 if we've worked together a year or more.

How much do you usually spend on a nanny's birthday? She's been with us for 4 months and for DCs birthday she was generous and got presents for all the children (not just the birthday child's). I don't really know what she likes or needs and don't want to get something she doesn't like so I was thinking of a nordstroms gift card. The question is how much to spend.
