I needed a one day a week nanny (because we have an au pair) and it's a longer story than this but DS now goes to a nanny share one day a week with another toddler his age. Has worked out great - the nanny gets a little extra money, and we slightly defray costs. Even better, since she's a full-time nanny if the au pair is ever sick or on vacation he has a place to go.

I agree with this. I'm looking for this exact type of arrangement, and 10-12 hours per week is about right. You should be able to find someone if you are looking for a few early morning or evening hours. If you need 10 hours in one day, it might be more challenging.

Of course the extra time would be negotiated in advance to fit in with their schedule and would be paid accordingly. Probably just be the odd evening. I am not looking to have someone around to be on call instantly.

That's a lot to ask for. The one day a week? Sure, it isn't unheard of for a nanny to work a 4 day per week gig.
But if you expect her to be on standby when your husband travels....would any of these times be during a normal workday? I can't imagine any instance where someone is available to work one day per week and has no other commitments other days. If these other times are at night it's a possibility, but I personally wouldn't take a job offer like that unless it was in the work agreement that x amount of notice would be given before I'm expected to work the extra time. For example, if we sat down at the start of every month to schedule any 'extra' days I'd have to work.

You need to document the extra sitting as required. The live in might not be available as often as you like

Ideally you could find someone who is only using a nanny 4 days a week, and then you would have a perfect combination for someone. Try asking around all your listservs, ask your real estate agent to do the same, etc...
