How much are you going to give her for the spa? IMO, you can't really go to a spa and spend less than $200. That's a lot to spend on a birthday gift for your nanny.

I'd do a spa gift certificate, and if you can, coordinate the visit on a day you can come home early to take over the kids.

I think cash is tacky for a birthday present.

I've received, cash, dessert, a gift card, tickets to a musical for my mother and I, and a massage at a nearby spa. I enjoyed all of these things.

You are wonderful family,I have been a nanny for 22 years and I never had anything for my birthday,last June was my bday,my boss even say to me happy birthday,I was on this time very,very sad,I hope one day I get one family like yours.congratulations!!!

OP, spa day sounds wonderful, but I would give her a choice of dates rather that selecting the date yourself. What if she'd rather spent a free birthday evening with her family and then went to a spa on a weekend?

Nanny here - with my previous family I always got $ (my weekly pay), cake, flowers, handmade cards from the kids and some little gifts "from the kids" - gift cards, Starbucks card (my favorite!!), chocolates, etc.
At the end of the work day, usually an hour or two before I was done for the day, MB asked what I wanted for dinner, so we ordered takeout from my favorite Italian restaurant and then had a little celebration/dessert with cake and a glass of wine.
I absolutely loved it and appreciated it every year
