BCBS through MHIP
$269/month (we pay full premium directly to the insurance provider)
Great policy, covers a lot and has a good prescription drug plan

My employers give me $150/month for healthcare.
My insurance costs $220/month, so I cover the difference.
It covers things that health plans cover - not going to break down all my copayment and deductibles here though.

That's way too broad of a question. Some families pay for their nannies policy whereas others give a health insurance contribution each month per the nannies choice. Policies can range from $150-600 a month depending on the policy and coverage. In my ten years of nannying, I've never had a family pay for my health insurance. Frankly, I wouldn't want them to because I'd be getting a crap policy with extremely high copayments.
