Here's what smart parents do: They know their child. If their child (that they know) wasn't well-cared for, they'd know it.
So I was a first time mom. My 1 year old came with diaper rash 0-3 times a month especially during summer. Provider said she checks for soiled diaper every 30 min. I had no idea to gauge whether my child was being neglected or not. I stayed. I've seen the provider play with her phone at times and my then 18 month old put my phone on her ear and played phone and learned to say the word phone as one of her first 5 words. We had never taken a call in front of out toddler so I suspected that she learned how to hold a cell properly to one's ear from her provider. We still stayed. Now my child is 30 months and can say I like Anna, I no like Maria, but how would a first time parent have known before your child starts talking on borderline situations where it's not apparent neglect??
