You absolutely have the right to voice your concerns with the nanny but it doesn't seem like there is much that she can do right now. You can take the situation into your own hands and pay to have a mechanic inspect the vehicle and let you know if it is safe enough to be driving children around in.

You can see if there are any additional services that the nanny could do for you (longer days, evening hours, housekeeping) and offer her more money to do these things. Of course she may decline but maybe she will accept if it will make a car more affordable for her.

You can help the nanny pay for a new car and have a certain amount of money withheld from her paycheck each week to pay you back.
