LA nanny here- It's hot year round in the valley and we are outside all day. It's not humid here but if we can be outside in 110 degrees in the summer, your nanny can handle 20 mins. I suggest going out first thing in the morning. If you're asking her to take a still between 1-3pm when it's hottest and horrible I can see why she doesn't like it but wtf it's only 20 mins!! We are outside all day every day.
Another LA nanny and I've got twin 2 yr old charges. We are outside all day everyday! When the full summer hits we don't go to the park (not well shaded) but we opted for water play in the yard. In the mornings we are outside for at least an hour. In the afternoon after nap, we can stay out till dinner. The kiddos love hiking right now, so we do lots of basically level hikes in the woods, they love nature. I couldn't fathom staying in doors so much!
