As for the transition, this is why shares are great— the two older ones have each other to interact with while nanny feeds and diaper baby.  If she’s creative, she’ll still find ways to include them in even simple (or quick task).  It’s definitely doable and if your nanny says she’s comfortable and capable, then trust her and not those who’ve never been in that situation— I’m surprised no one has chimed in with the “oh, that’s impossible” lol.
