I doubt paying $32/$48 for OT is not going to guarantee quality care for these children.  Better to hire a part-time helper for a while for the busier part of the day (mornings?) to assist the nanny.  That way, the older child can go to an activity once in a while and they might be able to go to a park within walking distance all together (unless one of the families is providing a vehicle with three car seats).
