I'm not a nanny but I have had the experience of knowing in the first week that I made a terrible mistake taking a new job.  I couldn't afford to quit and it took me 6 months to find something else but they were six miserable months and my gut instincts from the first week were dead right.

You sound like an experienced nanny so you should trust your gut.
