Yes, this is the scary part of giving several month's notice - you could lose your nanny before you're ready to be without childcare.  OP, for this reason if/when we have to part with our long-term nanny we will err on the side of significant severance.  So perhaps we'd give her one month's notice from the time we're ready to be without her, but 2 month's severance pay so she has ample time to find her next position without anxiety.  It's expensive but it's the best way I can think of to ensure we don't get left in the lurch and she gets the consideration and peace of mind she deserves.

I also expect we'd be like the other poster and help our nanny find her next position.  She came to us from exactly that kind of situation (neighborhood referral when her time with them was coming to an end) and it was a terrific way to find a nanny.
