w2 boxes 15 - 20, leave blank? RSS feed

I employ a MD resident in DC as a household employee, am not withholding any federal or state income tax. I have made the w2 online through IRS business servies online, and have all the SS and unemployment done, but confused on boxes 15 - 20 regarding state and local wages... do I leave them blank or put wages in box 16 (state) and/or box 18 (local)? Also I only have a federal EIN, I called DC tax and they said there is no separate DC EIN, it's the same as the federal (not sure this guy was the brightest bulb so please correct if wrong)... but would box 15 be state where wages were earned (DC) or state where employee lives (MD)? Appreciate any help!!

The State box will be MD and the taxable state wages are almost always the same as Box 1. There is no local wage - in MD they bundle the state and local wage into one collection/tax document.
