Op, I've seen you asking this in several forums. The DMV is a HUGE market for au pairs. Most people in this area, if they are seeking live- in childcare, it's usually an au pair for the affordability and flexibility.

Pretty much all agencies place live-in nannies. I don't know of any that specifically specialize in it. I actually don't even know of any agencies at the national level that "specialize" in live-in nannies. There are many, many more live out nannies than live in. My assumption would be that every agency handles live in placements to a rather similar ratio.

Why would any agency want to limit, or “specialize” in only live-in nannies? That’s ridiculous, OP.
