Any recommendations on where to find a good template and where to run background check?

There are numerous contracts available if you run a google search. The background checks that agencies run is the same type that care and sittercity run, but you have the ability to see everything on the agency's report (if they show you). If you'd rather, you can do any reputable background check and get the same computer-run search done.

Use a finger print background check, OP, I’m not in DC but I’m sure you have several agencies that provide them. Use whichever company foster Parian’s school teachers use.

Online social security backychecks are pretty useless since the social security number could be fake.
