So it seems liked McCarthy is agreeing to all kinds of concessions in the rules package, giving chairmanships and committee seats to various hold-outs, etc. How will he actually follow through on that? Don't the rules package and the committees have to get voted through as well? It seems like the 200ish members voting for McCarthy aren't going to be keen on giving the crazies what they want.

I think you have it backward. They have zero motivation to move the vote forward more quickly, they know how it's going to go. The main reason they are even having a vote is for the opportunity to make speeches.
